Consent form for 1st Dec and 22nd December

Note:

1. Print the consent slip out, sign and hand it on the 1st of Dec.
2. Top up ez link cards before 1st Dec.
3. To have some money for lunch as lunch will only be partially subsidised.
4. Bring at least a 1.5L water bottle.
5. Bring a poncho (recommended) OR umbrella.
6. Bring at least a digital camera (if really no choice, then camera phones)

>For those who are unable to print the consent slip out and sign it, their parents need to personally write a small acknowledgement slip and sign off (with date). For those who have the personally written acknowledgement slips and those who have nothing, their parents will be called on the day itself to make sure their parents know that they are with us, going out of school.
